Maybe chronic compartment syndrome?

Tricky patient, any advice on things to check is appreciated. 35yo female 7 month history right shin pain following tib ant and puns and needles. No trauma unsure of cause. Started after having a week off of work then came back in (works in a building trade centre wears steel cap trainers quite light) sudden severe pain was unable to walk more than 100 meters. MRI showed mild effusion over the tibia but nothing more. Only thing that could bring on the pain was heel walking for a minute or tib ant/fib palpation. No trigger points. Neuro clear, slump with peroneal and sural bias clear. Was a little tight in ant shin so worked on stretching and strengthening and over a month the pain essentially went. She stopped the exs and in 3 days the pins and needles was back and the pain was just starting. We started back on the plan with progressive loading on d/flx stretching and supportive foot ware. My thoughts were maybe straining due to heavy shoes but she brought them in and there light and flexible. So. I’m stumped as to what it is. Any ideas or things to check?
